Impact Health Policy Partners successfully led the preparation of Hackensack Meridian Health’s application to participate in the Accountable Health Communities Model pilot program. Hackensack University Medical Center is one of 32 organizations nationwide selected for a Centers for Medicare & Medicaid Services award that will identify and address the health-related social needs of Medicare and Medicaid beneficiaries in the community.

“The grant addresses the Medicare and Medicaid population and screens to identify gaps in social and medical needs,” said Lisa Tank, M.D., vice president of Medical Affairs and chief of the Division of Geriatrics at Hackensack University Medical Center. “Patient care navigators will be crucial to assist the high-risk population in accessing appropriate services.”
